The Assessment System converts all of your agency’s paperwork to the computer, while retaining the look and feel of traditional paper forms and records. We provide a library of more than 600 different forms and assessments, which you may use as-is or change as needed. Our intuitive form-building utility can precisely replicate your current paper format.

Because the Assessment System is linked with the Anasazi Treatment Plan System, any information completed on an assessment form is automatically updated on the client’s Treatment Plan. This prevents important information from being overlooked during the course of treatment and ensures that problems identified during an intake actually make it to the treatment plan.

Once a service has been performed, the clinician checks it off the Treatment Plan and types in a progress note. This automatically begins the billing process, so there is no need for additional data entry or triggers. Also, our system prevents the billing of services for which there is no documentation, protecting your agency against Medicaid fraud audits.

The Progress Indicators Feature allows your organization to measure treatment effectiveness (i.e., a client’s GAF score over time). And with the Laptop Assessment and Treatment Plan System, your clinicians can download their client files to a laptop and update the forms in the field.
